DOT Physical Regulation Change
Effective May 21st, 2014 all Department of Transportation (DOT) physicals must be conducted by a certified and registered medical professional by the Federal Motor Carrier Safety Administration (FMSCA). Chad Davis PA-C is our provider who is certified by the FMSCA to conduct these physicals. Due to additional requirements performed at this type of physical, it is important to notify the scheduler that the appointment will include a DOT physical.
TDaP Vaccine Requirement
The Iowa Department of Public Health is requiring a TDaP (tetanus, diphtheria, and pertussis) vaccination for all students entering into 7th grade. Many students have already received the vaccination and simply need to provide proof to the school. Please call our office to check and see if you child is up to date on this immunization.
